Connected ECG in telemedicine: everything the remote physician sees

In basic telemedicine — a video call — the physician cannot see an ECG tracing. They can ask the patient to describe chest pain, but without data they cannot rule out an acute coronary syndrome. A connected ECG in telemedicine fundamentally removes that limitation.

When the nurse places the electrodes and runs a 12-lead ECG connected to the MedConnect platform, the remote physician sees the full tracing in real time on their screen. They interpret, decide, and prescribe — with the same data they would have in the office.

Why a 12-lead ECG changes everything in teleconsultation

The connected electrocardiograph is the device that most radically transforms the quality of a teleconsultation. A 12-lead ECG provides a complete map of the heart's electrical activity — not just a single-lead tracing like a smartwatch.

With 12 leads, the physician can:

  • Detect a myocardial infarction (ST-segment elevation)
  • Identify atrial fibrillation
  • Spot a bundle branch block
  • Assess ventricular hypertrophy
  • Monitor the effect of antiarrhythmic therapy

Without an ECG, a physician in teleconsultation can only send the patient to the emergency department "just in case." With a connected ECG, they have the data to decide.

What the physician actually sees remotely

On the remote physician's screen, through the MedConnect platform, the ECG appears as follows:

  • Real-time 12-lead tracing: all 12 tracings display simultaneously while the nurse performs the exam. The physician watches the recording build live.
  • Glasgow algorithmic pre-interpretation: the Glasgow algorithm built into the ECG device (Cardioline Touch HD+ or Schiller) provides an automatic analysis. It flags potential abnormalities — irregular rhythm, abnormal ST segment, prolonged QT — before the physician even reads the tracing.
  • Archived PDF: the full tracing is automatically converted to PDF and archived in the patient record, available for any future consultation.
  • Tele-expertise referral: in one click, the physician can send the tracing to a cardiologist via Omnidoc for a specialist reading.

Cardiomatics AI analysis: remote Holter ECG

For long-duration recordings (24-hour Holter ECG), MedConnect integrates Cardiomatics — an AI-powered ECG analysis service available through the RESAH partnership (RESAH is a French public-hospital purchasing network).

Cardiomatics analyzes Holter recordings and detects:

  • Atrial fibrillation
  • Ventricular and supraventricular premature beats
  • Sinus pauses
  • Heart rate variations

In a nursing home, this means a resident can wear a Holter monitor without leaving the facility. The recording is sent to Cardiomatics through the platform, and the analysis report comes back into the patient record. The coordinating physician and the cardiologist share the same data.

Clinical cases where a connected ECG makes the difference

Suspected acute coronary syndrome (ACS)

10 p.m. in a nursing home. A resident complains of chest pain. The night nurse performs a 12-lead ECG at the bedside with the telemedicine cart. The on-call physician sees the tracing in real time: no ST elevation, normal sinus rhythm, stable vital signs. Decision: monitor on site, recheck the next day. Transfer avoided.

Incidental atrial fibrillation

During a routine teleconsultation, the ECG reveals asymptomatic atrial fibrillation. The physician requests a cardiology opinion via tele-expertise. The cardiologist receives the tracing through Omnidoc and recommends starting an anticoagulant. The resident never left their bed.

Post-MI follow-up at home

A post-myocardial-infarction patient is followed at home by a home-care nurse. At each visit, she records an ECG with the telemedicine kit. The tracing is compared with previous tracings in the patient record. The treating cardiologist accesses the data through the platform and adjusts treatment as needed.

Cardiology screening in a primary care network

A general practitioner orders an ECG. The nurse at a multidisciplinary health center performs it with the connected equipment. The tracing is sent to a cardiologist for tele-expertise through MedConnect telecardiology. The patient gets a specialist opinion without a cardiology appointment or a trip.

FAQ

Is a connected ECG as reliable as an in-office ECG?

Yes. The integrated devices (Cardioline Touch HD+, Schiller) are CE-marked 12-lead ECGs equipped with the Glasgow algorithm — the same standard used in physician offices and hospitals.

Does the remote physician see the tracing in real time?

Yes. The tracing displays live on the physician's screen while the nurse performs the exam. The physician can ask for an electrode to be repositioned if the signal is noisy.

Can the tracing be sent to a cardiologist for tele-expertise?

Yes. From the MedConnect patient record, the physician sends the ECG tracing in one click via Omnidoc. The cardiologist receives a high-resolution tracing and returns their interpretation. In France, the act is billed under code RQD (€10).

How much does a MedConnect-compatible connected ECG cost?

From €2,299 for the 12-lead ECG device. Lease financing is available. The device is included in the standard configuration of the telemedicine kit and cart.
